Use this service catalog to request a new Siphon account. Please answer all questions to the best of your ability.

Steps needed for Siphon access:
1. Submit this Siphon - Support Request to generate a MHSSHD ticket.
2. Each user will need to complete a Siphon 2875 Form. We have completed as much of the attached form with our information and will need additional data and signatures from the user and their Supervisor. Instructions are also attached.
3. Additionally, we will need Appointment Letters (sample letter) for all Approvers. Submitters do not need to provide an appointment letter, only a Siphon 2875 form.
4. A Siphon New User training will be scheduled once we have your completed forms. This training is required for Siphon access.
5. We will provision your Siphon account after you have completed your Siphon New User Training.
6. Please fill out this New Site Questionnaire if you are a new site.
Warning: To prevent a system time out. Please fill out all of your required document attachments and have them ready to go prior to filling out this request form.
For your reference, there are two roles in Siphon - a Submitter and an Approver. In short, the submitter configures the change request or connection in Siphon and the approver reviews it before approving for implementation with the firewall team.
Typically, Submitters are System Admins and Approvers are an ISSO/ISSM or CIO - but not always the case. We will discuss the roles more during the Siphon New User training.

If you already have a Siphon account and are having access issues, please be sure you are using Google Chrome, and not Internet Explorer, to login via siphon.health.mil.
If you are still unable to login to Siphon, your account may have been deactivated due to inactivity. Please submit a MHSSHD ticket and request to have your Siphon account reactivated.
Siphon accounts are automatically deactivated after 90 days of inactivity, so please be sure to login regularly.
